I certify that my information is true and complete to the best of my knowledge and do hereby authorize and give consent to the Brick Super Novice Invitational Tournament, its agents to inquire into and undertake whatever background check Brick Super Novice Invitational Tournament deems appropriate. I understand the inquiry may include a police information check, computer database searches, interviews with people acquainted with me, employers or references. I understand that the information will be kept confidential in accordance with the laws of Alberta in accordance with the Personal Information Protection Act.
